2. Maternity. [draagmoeder] is hereby declared to be the birth and legal mother and guardian of the minor child, [minderjarige] , born on the [geboortedatum 2] 2024 in [geboorteplaats 2] , Minnesota from birth to the date of this order. [donor] is hereby declared not to be the intended or legal mother of the minor child, and any existing presumption of maternity and/or legal rights that she may have to the minor child are hereby expressly and immediately terminated form the date of this order forward.
3.Legal Name. The minor child’s legal name is hereby established to be [minderjarige] .
4.Custody. Prior to the date of this order, [draagmoeder] has remained
